What to Expect from Your SUD Assessment
Your assessment appointment is a structured clinical interview to evaluate substance use patterns, history, and treatment needs. Kim will gather information about your substance use, mental health, medical history, and current life circumstances to provide a comprehensive assessment report. This is an assessment-only service; ongoing treatment is arranged separately if needed.
Who This Service Is For
SUD assessment-only appointments are designed for adults (18+) who need a comprehensive substance use evaluation for court requirements, probation documentation, CPS cases, treatment planning, or personal insight. This service provides the clinical assessment without requiring enrollment in ongoing treatment.
Fees & Insurance
We accept Medicaid (PMAP, MA, MinnesotaCare) and offer sliding scale options for self-pay clients. Assessment fees will be discussed during scheduling.
About Kim Craig, BS, CPRS-FE, LADC
Kim Craig is a Licensed Alcohol and Drug Counselor (LADC) and Certified Peer Recovery Specialist - Family Engagement (CPRS-FE) specializing in harm reduction approaches and trauma-informed substance use disorder treatment. With a deep understanding of the complexities of addiction and recovery gained through both lived experience and professional training, Kim brings authenticity, compassion, and evidence-based practice to assessment work.
